Reaching Out, Making the Connection.

We know that reaching out for help can be the most difficult task of all. That's why we want to make sure that whatever your needs, you can make the connections with Cape Counseling. The Access Center is the overlapping central point of entry for services provided by Cape Counseling or other appropriate services provided in the Cape May area. One phone call is all it takes. The scope of services includes:
  • 24 hour hot line coverage
  • Clinical assessment, referral and linkage
  • Consultation, training and technical assistance to other clinical staff
  • Referral to appropriate outpatient programs, and community services.
  • Referral to inpatient care if necessary.
  • Telephone screening.

Integrated Case Management Services (ICMS) is a program at Cape Counseling Services that was established in 1997 to help persons with mental illness transition from an institutionalized setting into the community.  The program is primarily comprised of six caseworkers who provide intensive in-home support and assistance to individuals who have been diagnosed with a chronic mental illness and/or have been recently discharged from a state or county psychiatric hospital. The role of an ICMS case manager is widely defined because of the array of responsibilities these services encompass.  Services include but are not limited to appointment coordination, linkage to mental health and medical treatment, assessment, collaborative contact with relatives and other professionals, comprehensive care planning, referrals, community support, crisis intervention, legal accompaniment, vocational training, and advocacy.  The ultimate goal of the ICMS program is to promote empowerment, independence, and stability so that our consumers can successfully integrate into the community and overcome the many barriers associated with mental illness. 

PACT (Program for Assertive Community Treatment) provides comprehensive treatment to people with serious and persistent mental illness. People served by PACT receive individualized multidisciplinary services that include staffing available 24 hours a day/ 7 days a week, and a staff Psychiatrist. PACT provides services to people in their own community and homes as well. PACT staff members are trained in nursing, social work/counseling, substance abuse, vocational counseling, and the promotion of peer advocacy.

Adult Partial Care (APC) assists individuals who have severe and persistent mental illness with developing the skills and support necessary to live, learn and work with the least amount of intervention from the behavioral health system.  We offer comprehensive, individualized and quality behavioral health treatment within a group setting, designed to promote maximum independence in the least restrictive setting.  Our goal is to help people who have experienced set backs due to a severe mental illness regain a meaningful constructive sense of membership in their communities.   We accept Self-referrals and referrals from physicians, hospitals, other health care professionals, parole and probation and from families. 

Adult Residential is a transitional residential program for clients with chronic mental illness who have been discharged from Ancora Psychiatric Hospital. Residential services include: development of skills needed to reside in community such as budgeting, meal preparation and household maintenance skills.  Staff accompany client on medical appointments, designated residential nurse with on call responsibilities.  Nutritional education to promote healthy lifestyle practices.  Recreational activities provided within program setting and in community; development of social skills.  For those interested assistance in pursuing vocational opportunities such as educational courses or part time employment. .  The primary objective of program is to assist clients in the development of their skills for successful transition back into the community.

Recovery Services at Cape Counseling is dedicated to providing quality care for those suffering from substance abuse and the disease of addiction.  To this end CCS offers a full range of services at the outpatient level to address abuse and addiction chemical issues with both the adolescent and adult.  The goal of care is to intervene in the progression of the disease of addiction in an atmosphere of compassion, safety and understanding.
These clinical services include drug and alcohol awareness classes, outpatient one day a week recovery groups, intensive 9 hours a week group counseling, and weekly relapse prevention groups.
For those who need a higher level of care in a residential setting, a placement specialist in the Recovery Services program will assess, place and follow-up in care persons who need detoxification, residential rehabilitation and half-way house services.  This placement service is dedicated to remove the financial barriers in accessing care so that treatment is available and affordable.

Supportive Housing Program assists clients with housing needs, and provides support in the community to help clients obtain and maintain safe affordable housing. The program serves clients with mental health issues, youth aging out of foster care, homeless people, HIV/AIDS clients, and those challenged by co-occurring disordered (substance abuse and mental health). We obtain and manage state and federal vouchers for clients who are eligible, and provide those clients with case management services.

Mental Health IOP (Intensive Outpatient Program) Transition Towards Wellness Cape Counseling Services, the primary community mental health agency in Cape May County, NJ, now has a mental health intensive outpatient program available.  This is a 3-4 day per week program, 3 ½ hours a day which utilizes groups, individual therapy, psycho-education and medication monitoring to work towards wellness and recovery.  Transportation is available when needed.
In order to be eligible for the program, clients must be 18 years or older and a resident of Cape May County.  They must have an Axis I mental health diagnosis, through they may a secondary substance abuse diagnosis as well.  Clients should present with a high degree of clinical risk as evidenced by recent hospitalizations or multiple screenings.  This is not a partial hospitalization program.

Outpatient Services at Cape Counseling believes in and provides a multi-disciplinary approach to care.  This means that for any individual in any of the many programs, there is access to a team of professionals most qualified to meet their needs and facilitate a successful therapeutic process.  The family of professionals at Cape Counseling includes:

  • Advanced practice nurses.

Psychiatric Services at Cape Counseling Services provides psychiatric services through a contract with the University of Medicine and Dentistry of New Jersey (UMDNJ), Atlantic Care Behavioral Health and Cape Counseling Clinical Nurse Specialists. Services include:

  • Outpatient, both mental health and substance abuse.

Psychiatry adheres to the biopsychosocial model of care in which treatment is provided by a multidisciplinary team.  Psychiatry involves the client, and whenever possible, their families, in the development of the client’s individualized care plan.  Psychiatric Services is sensitive to a client’s cultural values and affords every client dignity, respect and the right to be treated in the least restrictive setting.  Treatment is provided in the most efficient manner while utilizing the use of community supports.

Intensive Family Support Services (IFSS) is a county-wide support program, administered by Cape Counseling Services, for families or individuals with a loved one with mental illness. The program is based on the belief that, despite recent improvements in treatment and community services, family members often take an active role in care for a mentally ill relative. This can be an especially difficult and challenging task. IFSS provides tangible coping skills to address these challenges. IFSS is open to all Cape May County families with a loved one with mental illness; the family member does not have to be in treatment for the family to receive services. IFSS was designed with extensive input and assistance from families caring for relatives with a long history of serious mental illness.
When your loved one is diagnosed with a serious mental illness such as schizophrenia, major depression or bi-polar disorder, finding appropriate medical and psychiatric care is your first priority. But once treatment begins, you and your family member soon discover that the illness affects many aspects of life and you need more than medical help.

IFSS is a comprehensive program of supportive services and activities designed to assist families with a mentally ill relative to:

learn more about mental illness and the treatments available
understand the community mental health system in Cape May County
acquire skills to help you cope with the behavior of your loved one. 
participate in a support network of families coping with mental illness
take a break from caring for your family member while being sure they are well cared for

MONTHLY SUPPORT GROUPS: Members of families coping with mental illness meet together with a mental health professional for mutual support and information exchange.

INDIVIDUAL FAMILY CONSULTATIONS: Family members meet with a mental health professional to develop strategies for coping more effectively with problems resulting from their relative's illness.

RESPITE CARE: Families who have their loved one living with them may qualify for planned time away.  An afternoon, evening or overnight break can restore emotional equilibrium, regain social connections, and renew strength.
EDUCATIONAL WORKSHOPS: Informal programs provide up-to-date information about mental illness and treatment options and also teach skills that enable caregivers to better cope their loved one's illness and improve the functioning of the family.

ADVOCACY: Family members learn effective ways to obtain and maximize available resources.
REFERRAL SERVICE: IFSS staff assist families in locating additional relevant programs and services.

Project for Assistance in Transition from Homelessness (PATH), an innovative program offered by Cape Counseling Services, Supportive Housing Division, is an intensive case management service which provide mobile outreach services to the psychiatrically disabled homeless living in the community.  This program is designed to reach out to consumers who have not accepted traditional center-based mental health services.  This service includes:

  • supportive services
  • advocacy
  • housing assistance
  • mental health services
  • substance abuse services
  • transportation.

Each counselor functions as part of a team, and all consumers interact with all members of the team.
